Our grantee, Mills-Peninsula Health Services, shares details on its program Peninsula Circle of Care that supports older adults as they transition from the hospital to their home or another care setting. As the U.S. population grows older, focusing on how to keep older adults healthy at home and out of the hospital is an important part of improving health care.
