The latest issue of Health Care Innovations Exchange, an online resource from The Agency for Healthcare Research and Quality, features Moore Foundation grantee, David Dorr, MD, and the results of a seven-clinic pilot test of his Care Management Plus program. The program seeks to improve the delivery of primary care to high-risk older adults by combining care coordination services of care managers with robust electronic tracking systems.

Care coordination involves multiple settings and medical staff. The program’s informatics tools provide care managers with reminders on appropriate best practices and help them to document patients’ care plans.

Pilot test results showed that the program provided patients with greater access to needed services, which led to significant improvements in patient outcomes, including fewer complications, deaths and hospitalizations for diabetes patients. An increase in physician productivity and a reduction in medical costs were also reported, yielding savings of roughly $75,000 per case manager.
