As individuals age, they sometimes struggle to understand the role of a proxy as well as how to choose one to help them handle their health care needs. A health care proxy is the person chosen to make health care decisions for those who are too sick or unable to make them alone.

Foundation grantee, The Conversation Project, hopes to tackle this issue with the creation of the new guide “How to Choose a Healthcare Proxy and how to be a Healthcare Proxy.” This tools aims to help individuals understand the importance of designating someone to help them make decisions about their health, and how to choose the appropriate person to do so.

“We cannot overstate the importance of choosing a proxy. Too many people — including half of all the people over 65 who are admitted to a hospital — are unable to make decisions for themselves.”
