A new web-based portal used by patients and their caregivers while in the hospital has helped them to understand and take part in developing their own plan for care. Created by Brigham and Women’s Hospital through its Prospect project (Promoting Respect and Ongoing Safety through Patient-centeredness, Engagement, Communication and Technology), the team of researchers say the new toolkit has widened communication between patients, families and clinical providers, aiding in shared decision making and improving the safety and quality of the care provided.
