Purpose
In support of the implementation of a transitional care program at Stanford Hospital and Clinics for heart failure patients. The goal of the program will be to achieve and sustain a 30% reduction in the 30-day and a 15% reduction in 90-day all cause readmission rates for patients with heart failure.
