Purpose
In support of the development, implementation, and evaluation of a Randomized Controlled Trial to test the efficacy of a Primary Care Health Coaching intervention at the Mission Neighborhood Health Clinic in San Francisco, with the goal of improving outcomes for patients diagnosed with Type 2 Diabetes and/or Hypertension.
