Purpose
This grant to the University of Pennsylvania, School of Nursing supports the design, pilot, and evaluation of a new care delivery model for older adults with multiple chronic conditions--a combination of the Patient Centered Medical Home (PCMH) and the Transitional Care Model (TCM). The goals of the pilot are to demonstrate positive trends in patient outcomes, patient satisfaction, caregivers’ and health teams’ satisfaction, and to determine whether to proceed to a large scale study of the new model.
